Reorder "Title" or rename to "Subject" to clarify it's not personal title
In the new issue form, I read "Title" as personal title (e.g. "Mr", "Dr") first, then realized it was the issue title.
You could clarify this either by moving "Title" below "Type of issue" so it groups with the textarea, or renaming it to "Subject" or similar, or both.
Might be better still if you move "Name" and "Email" below the form, since when someone goes to the form, they probably want to dump their issue first, and worry about identity later (if at all -- posting without specifying details would be nice if it's not supported already).

Thanks for the report. Changed it to "subject". We need email so that we can contact the person about their issue.
