Mistreatment of a Patient by a Clinic in Prescott/Phoenix
Does your agency have any control over the manner in which a patient is treated at a medical facility? In my case, they accused me of not going along with their treatment plan, which is a lie. They cut me off my medication "cold turkey" and then prescribed a Withdrawal Pack, which, had I so much as taken one of those pills, I could have had a very serious, possibly deadly reaction. They have been lying their way out of all of this so far and they have not let me speak to their Administrative Officer in Phoenix. The Clinic is The Pain Center in Prescott, Arizona. They even falsified a form, claiming I would not sign that dismissal form.....which is a huge lie, since I never was handed any kind of form to sign. Maybe you can help me or guide me as to what can be done. Their lies always get them "off the hook"
I tried to attach a couple of emails I sent to their Patient Relations Director, but each time I tried to attach a second or third one, the others would disappear.

I tried filling out the form that is on that website, and then it asked for Attachments.....and I put 3 attachments.....and from the best I can tell, nobody at that site does anything.

Hi Brandi,
Guess I don't quite understand where your company fits into a situation like I've gone through with doctors here in Prescott. The Arizona Medical Board has sent me some papers to fill out, but I heard they really don't do much for people.

Federal Registrar uses our application as their customer helpdesk, so they are effectively one of our customers and we wouldn't be able to address your issue.
