Possible to add a custom form when a new discussion is opened as a default?

Despite having a knowledge base article "Found an issue - now what?" User rarely provide all the needed information.

So we thought it might be smart to add a default form (as seen here: http://support.gpgtools.org/kb/faq/found-an-issue). Would that be possible? If so, where can we set that up?

Thanks a lot for constantly improving Tender. It's really fun to use it and has helped us a lot with our support.

    Steve,

    This is a popular suggestion that we are definitely considering for future updates. I will add the ticket to this discussion so that we can update you if we make any changes. This wouldn't be until down the road as we are a very small and busy team!

    Hey Amanda, do you have a link to the ticket so I can subscribe to that in order to receive mails if status changes?

    This is much needed. We get so much feedback and troubleshooting would be so much faster, if we had the correct info right away. But people don't like to use the search function so we end up asking the identical questions over and over again...

    Since I'm no entp stuff, I assume I can't see the ticket you linked internally.
